For Students

    Use this form if you need to take the ACCUPLACER but do not currently have a UH Number.

For Instructors

Effective Spring 2022: Tests / proctoring instructions must be on file at the testing center PRIOR to students scheduling an appointment with us.

  • Test Administration Form (PDF, 166KB form-fillable) - revised 6/2023
    For Kapi'olani CC based tests/instructors: Complete this form and submit with your test materials to kcctest@hawaii.edu or hand-deliver to the Lama Library during regular business hours.
  • If you are submitting a proctoring request for a student with testing accommodations, DSSO Testing Cover Sheets (printed on yellow paper) are available at the Testing Center or Disability Support Services Office in 'Ilima 104.
